Foothill Family Clinic maintains medical records for each patient treated at our clinics. You will be asked to provide proof of your identification with a valid government issued photo ID prior to receiving your records.
To request a copy of your medical records, please see the following instructions:
- Complete the medical records release form (click to download form)
- Submit to Foothill Family Clinic via email at records@foothillfamilyclinic.com or fax it to 801-365-1033
Records needed for continuing care purposes will be sent directly to your provider. Requests for medical records can take up to 72 hours to process. A fee may be charged.
