Account Set Up

Each patient 18 and over will be set up in our system with his or her own account. Each account will receive a separate monthly statement. Notify our business office if you choose to have multiple people on one account. Dependents under the age of 18 will be set up on the parent’s/guarantor’s account, as directed by the guarantor.

Patient Check In

Insurance cards and co-pays are required at the time of check-in. A fee will be assessed to your account If you do not pay your co-pay. A photo-ID is required for all new patients. Your picture will be taken or scanned into our system for future identification. Any items, including collection activity and past due balances, are noted on your account and may be addressed with you at the time of check-in.

Payments

You can pay on your account at any of our clinic locations, on our website at Online Bill Pay – Foothill Family Clinic or through MyChart, or also by mail with cash, check or credit card. Credit card payments can also be made over the phone. It is the guarantor’s responsibility to see that your account balance remains paid and in good standing even though we submit insurance claims on behalf of our patients. If needed, you can work with our Patient Account staff to arrange a payment schedule. If at any point your account becomes more than 60 days past due, your account may be referred to a collection agency for collection. This may result in additional fees and a negative credit report. Once an account is sent to a collection agency, you will no longer be able to schedule an appointment with our doctors.

Outside Services

Some laboratory and x-ray services ordered by your doctor cannot be done internally and are processed by an outside entity. You will receive a separate billing for these services. Payment is made directly to the outside entity, not to Foothill Family Clinic.
